I'm not entirely sure when this one came along, actually. It's purportedly a part of the Original Mythos and I can't find evidence that debunks this, so I'll roll with it.
Thomas Nealy was a very minor character in the whole ordeal, however it's one of the only audio only Original Mythos stories I've come across, if not the only one.
In October 1989, Thomas' daughter, Jessica, went out to play with her friend alone. Later in the day the parents of the friend called him, stating that Jessica wasn't there with them either. He went out into the woods where they lived to search for her, but found a tall, emaciated man in a suit instead. They stared at each other for a while until he realized that it had no face. That was when he ran.
Nealy was later interrogated by a police officer about what had occurred that night. He concurred these same events, eventually breaking down when he started to yell about the same entity being in the room as them. The officer tried to calm him, however, not for long; the officer didn't see what was behind him, being the same being.
The fate of the two is unknown, however, it's obviously supposed to be inferred that the two are deceased.
